ATHLETE ELIGIBILITY

Student-Athletes must:

• be academically eligible per OCS & GHSA requirements
• be covered by medical insurance—School insurance policies are available through K & K insurance

The school accepts no responsibility for accidents or injuries in the course of practice or competitions.

• have parents complete and sign all participation paperwork on DragonFly MAX yearly
• have a current physical signed and dated by a medical professional and uploaded to DragonFly MAX

You must use the GHSA provided form.

ALL DOCUMENTS SUBMITTED TO RANK ONE MUST BE APPROVED BY THE ATHLETIC TRAINER PRIOR TO ANY PARTICIPATION. THEREFORE IT IS EXTREMELY IMPORTANT TO COMPLETE ALL INFORMATION AT LEAST A WEEK BEFORE TRYOUT EVALUATIONS AND/OR VOLUNTARY WORKOUTS.
